At WTCSB our mission is to provide a coordinated system of quality recovery-oriented care in Mental Health, Intellectual Disability, and Substance Abuse Services to the citizens of the cities of Franklin and Suffolk and the counties of Isle of Wight and Southampton. We are seeking Full-Time LPN positions for various programs throughout our organization.

• Implementing and maintaining Individual Service Plans
• Conducting ongoing physical and mental assessments
• Maintaining accurate consumer records, monitoring consumer safety
• Supervising and monitoring individual medical treatment programs
• Administration and delivery of consumers’ prescribed medication as needed
• Monitoring and education regarding medication compliance and side effects
• Assessing consumer’s vital signs, height, weight, medication compliance, and possible side effects, and obtaining AIMS score or laboratory tests as ordered by a physician
• Completing PA (prior authorizations) and patient assistance for consumer’s psychiatric medication needs
• Compliance with agency policies and procedures, and adherence to all relevant local, state, and federal legal and regulatory requirements

• Graduation from an accredited Licensed Practical Nurse Program
• Current Virginia unrestricted LPN License or Current Unrestricted LPN License from a Nurse Licensure Compact State
• 1-year LPN work experience in a mental health program and/or residential setting
• 1-Year LPN work experience involving individuals with intellectual disabilities
• Valid driver’s license with acceptable DMV record
• Proficiency in Microsoft Office.
• Preferred: Experience using Electronic medical records; basic knowledge of Medicaid/Medicare Regulations.
